Preparing for an FASD Evaluation
A complete assessment for fetal alcohol spectrum disorders (FASD) is complex, but our team is here to help. We will work closely with you to gather all necessary forms, documents and paperwork. Our comprehensive process reflects our commitment to providing an accurate diagnosis.

FASD evaluations can be lengthy and complex. These tips from our clinic team can help you prepare for an optimal experience:.
FASD evaluations are much longer than a typical doctor’s office visit. The medical evaluation may last 2.5 hours and the neuropsychological evaluation may last up to 3.5 hours.
In most cases, an adult must be present in the clinic for the entire evaluation. You are advised to notify work and school well in advance of the scheduled evaluation.
An FASD evaluation requires that our clinic team gather extensive medical records and other information from you. Our Care Coordinator will help you track down the required documentation.
To do this, the parent or legal guardian must sign the necessary releases of information and provide legal proof that they can sign them (for example, an adoption decree or guardianship order).
During the intake process, you will be asked to provide detailed information about your child’s medical and behavioral health history. This might also include school special education records.
For some evaluations, parents and teachers will be asked to complete questionnaires prior to the evaluation.
Parents will also be asked to facilitate the return of the teacher’s completed questionnaires to Proof Alliance.
